Terms and Conditions:

1. Vouchers are valid for 5 years only from date of purchase
2. When making an appointment, holders must identify themselves as voucher holders and quote voucher number at time of booking. Vouchers must be presented at time of appointment.
3. Vouchers are the sole responsibility of the holder
4. Vouchers are non transferable from the person it is brought for
5. Vouchers are transferable between services up to the value of the voucher
6. A full refund will be given if vouchers are cancelled within 30 days of purchase
7. Vouchers are non refundable if you fail to cancel within 24 hours of appointment date/failure to turn up for appointment.
8. If at appointment the service is deemed not suitable by the doctor the voucher can be transferred to another service or a refund will be offered. A consultation charge may apply.
9. Vouchers may not be resold.
10. Data collection and use are subject to the privacy policy of the practice.
11. The practice reserves the right to refuse redemption of a voucher and request alternative forms of payment if a voucher is fraudulently obtained or used.
12. Mullingar Medical Centre reserves the right to alter, change and/or add to the terms and conditions of the voucher at any time without notifying the holder.
